Health Services

The Licensed School Nurse and the parent/guardian and physician, will develop individualized health plans for those students who have specific medical needs and who may require medical attention during the school day. These plans are to be reviewed annually.

Please contact your school health office if your child has medical concerns that may require an individualized health plan.

Below you will find Individualized Health Plans that can be used for Diabetes, Severe Allergies, Asthma, Seizures and a generic plan that can be used for other medical conditions that require individualized attention.

• Diabetic

Note:  It may take several weeks to prepare an individualized meal plan with a completed special diet statement.  Parents/Guardians are responsible to provide meals from home for their student until a completed meal plan is in place. The Assistant Food Service Director, or  designee, will contact the parent/guardian with the proposed completed meal plan and anticipated start date. The school food service manager will contact with the parent/guardian with the actual start date.

If your child is evaluated to see if they are eligible for special education, and will receive Health Related Services as part of his or her Individual Education Program (IEP), the Burnsville-Eagan-Savage School District will share your child's name and date of birth with the Minnesota Department of Human Services (DHS) to find out if your child is on Medical assistance or MinnesotaCare. 

If you do not want the district to share your child's name and date of birth with DHS, you need to let us know in writing at least one week before we begin to do the evaluation. Every school year, students participate in grade level health screening. Students also receive hearing and/or vision screening upon request from their parents or guardians or if the teacher suspects that there may be a hearing or vision concern that is affecting the student's ability to learn. We also attempt to conduct a hearing and vision screening on students who are new to District 191.

The following is a list of the type of screening and the grade levels that are tested:

• Vision: Grades 1,3,5 and 7
• Color Vision: Grade 1 (males only) 

If your child fails any part of the hearing and vision screening following the MN Department of Health guidelines, the health office will re-screen the child 1-2 weeks later to verify the results. If the student fails the second screening, a letter will be sent home with the student outlining the findings and recommendation for follow-up with a physician.

If at any time you question your child's hearing or vision, feel free to contact the health office.

If you do not wish for your child to receive health screening, please inform your school health office.

The District 191 Wellness Committee consists of a broad representation of district and community stakeholders and is focused on student health and wellness initiatives in our schools, as related to nutrition, physical activity, social, emotional, and behavioral health.
